Our ADHD Screener will provide a probability that your child may have ADHD. It can be used to support a referral for an ADHD evaluation by the appropriate health professionals. 

Step 1

Prior to clicking on the registration button above it is essential that you read through the following 2 documents and ensure you are happy to accept their content:

Step 2

Click the purple registration button above to complete the necessary details and make payment for this service. 

Please note that all prices are initially given excluding VAT. 20% VAT is applied at check-out.

Step 3

After payment, you will be given a link to download the form you will need to complete. 

Step 4

Within 2 weeks, please email your completed form to amazing.children@potentialplusuk.org 

Please note that as per our Terms and Conditions of Sale above, refunds incur admin costs, and cannot be made after the delivery date of the report. This is up to 24 days when the form has not been received by us or less when the form has been received by us.  

Step 5

You will be emailed your screener result up two weeks after your form is received by us, as long as the form has been received within the delivery date.
