Step 1

It is essential that you read through the following 2 documents and ensure you are happy to accept their content:

Please also make sure that you choose the correct bundle for the screeners you wish to purchase, some screeners are only suitable for children 3+, 5+ and 7+ so please make sure your child is old enough for the screeners you wish to purchase:

The Autism and Social and Emotional Development screeners are only suitable for children 3 years old and older 
The ADHD screener is only suitable for children 5 years old and older 
The Anxiety, Depression and Self-Concept screeners are only suitable for children 7 years old and older 

Step 2

Click the purple registration button above to complete the necessary details and make payment for this service. PLEASE MAKE SURE THAT YOU SELECT THE CORRECT TICKET FOR THE BUNDLE OF TWO THAT YOU WISH TO PURCHASE.

Please note that all prices are initially given excluding VAT. 20% VAT which is applied at check-out.

Step 3

After payment you will receive 3 emails, please find the one with the subject: 'Potential Plus UK - Your downloadable items are ready' in the body of this email click 'Download Now' to download the forms/documents for the bundle you have purchased. 

Step 4

Within 2 weeks, please email your completed forms to amazing.children@potentialplusuk.org 

Please note that as per our Terms and Conditions of Sale above, refunds incur admin costs, and cannot be made after the delivery date of the report. This is up to 24 days when the form has not been received by us or less when the form has been received by us.  

Step 5

You will be emailed your screeners result up two weeks after your form is received by us, (this can take longer during school holidays), as long as the form has been received within the delivery date.

Finally, the safety of children is everyone’s responsibility and this is reflected in Potential Plus UK’s Safeguarding Policy. We work on the basis that safeguarding always overrides confidentiality as the welfare of the child is paramount.
